Paper quality and printing are good, but the form name is printed on the bottom of the front of the form. We can clearly see what the form is for and I don't think anything other than the form itself should be printed. Suggest removing. Shopping, purchasing and shipping were great.

Love the quality. Only wish they did not have lines, it was very difficult lining up print on lines. We type names and send through the copy machine. Otherwise pleased.
